Job Description:
We are currently hiring a Nurse Practitioner to join our
CloseKnit team to deliver hybrid in-person and virtual care.
CloseKnit is a virtual first advanced primary care practice looking
for an experienced provider interested in innovating in the
practice of primary care to deliver on the quadruple aim in a
value-based arrangement for adult and pediatric patients. While
providing both in person and virtual primary care services, the
nurse practitioner will be working with the CloseKnit team, to
optimize the care delivery strategy and best practices in
delivering a hybrid model of care to our patients.
