Independent Living Program Support (Temporary)

Location: Worcester, Massachusetts

This position supports two programs. The IL-LTSS program focuses on providing comprehensive support to consumers through face-to-face visits, conducting assessments, developing and monitoring care plans, and overseeing Long-Term Services and Supports (LTSS) delivery. These duties ensure integrated care management and ongoing assessment of consumer needs for effective service delivery. The NHI/OC program assists individuals transitioning from facility-based to community-based settings by helping with Home and Community-Based Services waiver applications, coordinating housing transitions, managing transitional resources, providing post-transition support, and offering Options Counseling to educate consumers and guardians about available services.

Key Responsibilities:

Conduct assessments and develop care plans
Assist with housing transitions and support services
Provide Options Counseling and education on available services

Qualifications: High school diploma required (Associate/Bachelor's preferred) with 1-2 years of experience in health or social services. Must have experience with community resources and service systems for persons with disabilities, including discharge planning, database management, and Independent Living skills. Knowledge of MassHealth, housing regulations, and disability rights required. Proficiency in Microsoft Office suite and experience providing Information and Referral services essential.

Requirements:

High school diploma required, Associate/Bachelor's preferred
1-2 years experience in health/social services
Experience with community resources and service systems
Spanish-speaking preferred

Physical Requirements: Must be able to sit for extended periods and lift up to 15 pounds
